Agreement of Participation with the Pennsville Babe Ruth League

As a Parent or legal guardian I give permission for my son to participate in Salem County Babe Ruth League Program. My child also resides within the boundaries of Salem County. I understand that I will be responsible for the purchase of my child's team pants, belt and socks as required by team manager. I also agree that my child will follow the League's code of conduct, rules, and regulations as well as Babe Ruth International rules and regulations.

In order to make immediate treatment possible for all players, in case of emergency or illness, I hereby authorize a representative of Pennsville Babe Ruth League to use his or her judgement in obtaining immediate medical care. In case of injury or illness, I will not hold the Pennsville Babe Ruth League responsible. I understand that I will be notified as quickly as I ca be reached in order to be made aware of an injury or illness.
