PARTICIPANT AGREEMENT:
Participants as well as their family and friends in attendance must respect the officials, coaches, team leaders and fellow participants. They must abide by the coaches decisions for the team and not coach the game from the sidelines nor subvert the coaches’ authority in any way. All issues, concerns or complaints are to be brought to the league coordinator or recreation director. Any player, parent or spectator displaying unsportsmanlike, violent, or profane behavior may be removed from the game, practice, activity, and/or the premises, at the official’s or GCBC; ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL Recreation/Competitive staff’s discretion. Repeated incidents or just one serious infraction can result in parents, guardians and/or participants being expelled from the GCBC; ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL Recreation/Competitive program without any refund of registration or fund raising money.
For the safety of yourself/your child and other patrons all participants are required to follow dress codes for the particular sport or activity that they are participating in. The required attire will vary from sport to sport, therefore it is up to you to discuss with your assigned coach or instructor the dress code requirements for your particular activity. For all sport activities, participants are not allowed to wear any jewelry, watches or other attire with metal that could injure another participant.
I consent to allow my or my child’s coaches or other trained adult to provide basic first aid in the event of injury, and to tape or bandage joints as appropriate for the sport of participation. I hereby give consent for a GreenForest Community Baptist Church (GCBC); ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL representatives to make transportation arrangements or take me or my child/children for care, medical or dental, in an emergency situation where our condition represents a serious or imminent threat to his/her life, health or well-being. I understand that a conscious effort will be made to notify me prior to such action and the expense, if any, will be borne by me.
I understand that GreenForest Community Baptist Church (GCBC) is a Christian organization and as such is mandated by the Bible to share the message of Jesus Christ with the community. I am also aware that GreenForest Community Baptist Church members routinely share the Christian gospel message at campus events, activities and performances including but not limited to games, practices, camps, clinics and training sessions.
I certify that GCBC; ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL may use, reprint, or reproduce names, photos and video taken of me, my child, my family and their likeness for use in but not limited to GCBC; ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL displays, publications, website, commercials, and promotions.
I understand that if I desire a refund, I must submit a written request for refund to applicable department no later than 7 days prior to the start of the initial practice or classes. All refunds are assessed a $25 processing fee per registration. No prorated refunds are given for partial sessions or sports participation and participant substitutions are not allowed. In the event that the registered sport or activity is cancelled before starting, then applicable department will reimburse the full registration fee.
MEDICAL AND LIABILITY RELEASE:
I, for myself, assigns, heirs, next of kin acknowledge and those under my guardianship agree that I understand the nature of sports activities and that I am qualified, in good health, and in proper physical condition to participate in such activities. I further agree and warrant that if at any time I believe these conditions to be unsafe, I will immediately discontinue further participation in these activities. I fully understand that participation in sports activities carry with it certain inherent risks of serious bodily injury that cannot be eliminated regardless of the care taken to avoid injuries. The specific risks vary from one activity to another, but the risks range from 1) minor injuries such as scratches, bruises and sprains 2) major injuries such as eye injury or loss of sight, joint or back injuries, heart attacks and concussions to 3) catastrophic injuries including paralysis and death. I understand that these risks and dangers may be caused by my or my child’s own actions or inactions, the actions or inactions of others and/or the condition in which the activities take place. I understand that there may be risks and social and economic losses either not known to me or not readily foreseeable at this time and I fully accept and assume all such risks and responsibility for losses, costs and damages that I may incur as a result of participation in sports activities.
I hereby release, discharge and hold harmless GreenForest Community Baptist Church (GCBC); ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL, their respective owners, lease holders, administrators, directors, agents, officers, members, volunteers, and employees, other participants, sponsors and advertisers from all liability, claims, demands, losses and/or damages caused, or alleged to be caused, in whole or in part by me or by my assigns, heirs, next of kin, and those under my guardianship. Furthermore, I will indemnify, save and hold harmless GCBC; ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL from any litigation expenses, attorney fees, loss, liability, damage, or costs which may be incurred as the result of such a claim. Furthermore, I will not hold the GCBC; ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL staff, volunteers or any other parent/ guardian responsible for injury in which me or my child may be incur as a result of traveling to and from GCBC; ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL sports and recreation activities.
In consideration of GCBC; ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL making available to me or my child the ability to participate in a sport program, use facilities and equipment, I waive, release and hold harmless GCBC; ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL, its agencies, officials, employees, recreation and volunteer staff or any other parent/guardian from and against any and all liability arising out of or in connection with me or my child’s use of the GCBC; ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL facilities, classes, programs and/or equipment. I also understand that while the GCBC' ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL staff or volunteers may provide general orientation as to the use of the recreation facilities and equipment the use of the same remains at my sole risk of me and my child which I expressly assume. I understand that GCBC; ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL does not provide medical insurance for any person participating in any sports program or using the church’s facilities and equipment, and that I am responsible for obtaining my own medical insurance which covers me and/or my child’s activities.
By my signature I declare and represent that I understand that GCBC; ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL staff, and volunteers conducts no medical examination or assessment concerning me or my child’s physical or mental condition and that it is solely up to me, in consultation with my or my child’s own physician or other advisor, as to whether or not me or my child will participate in the program or any game, practice or training session. I hereby certify that the participant that I am registering (me or my child) is in normal health and capable of participating safely in GCBC; ATLANTA AZTECS; BASKETBALL 4 ALL; SPORTS FOR ALL sports and recreation programs.
I have read this agreement, fully understand its terms and have signed it freely and without inducement. Shall any portion of this agreement be held to be invalid the balance, notwithstanding, shall continue in full force and effect.
