As the parent/guardian of a participant of the Fokus Baseball program, I recognize and acknowledge that there are certain risks of physical injury that may occur and I agree to assume full risk of any injuries, including death, damage or loss which I or my minor child/ward may sustain as a result of participating in any and all activities connected with or associated with such a program. I agree to waive and relinquish any claims I or my minor child/ward may have as a result of participating in the program against Fokus Baseball including any of its officers, agents, servants, coaches and employees, as well as any of the before mentioned individuals associated with the Bolingbrook Park District.
