Waiver Information

I hereby release the Unionville Ladies Slo-Pitch League (ULSPL) from any liability for injuries or damage sustained while attending or playing games during the current season.

I am aware that by registering for the ULSPL 2026 season, I am agreeing to make a commitment to the league and my team:

1. I agree to give more than 24 hours notice to my Captain/Co-Captain of absences.
2. I understand that on my 5th absence, I will be given a warning that attendance is an issue.
3. I understand that after my 6th absence, I may choose to resign my playing position and remain a Member in Good Standing for the next season (automatic return to the league).
4. If I decline to resign after my 6th absence, on the 7th absence, I will be declared Not in Good Standing and my membership in the ULSPL will be revoked.
5. I acknowledge that in order to play and/or sit on the bench, I must register online with SPN and sign a waiver for insurance purposes. Emails will be sent out after team selection in the Spring.
6. City of Markham residents will have priority over new non-resident registrations.
7, I agree to abide by the Code of Conduct as stated in the ULSPL constitution.
