Player Registration Form for 2026 Bellaire Baseball SAC Camp. SAC Camp is a BHS / UIL approved camp for all student athletes grades 7 -12, participating in Bellaire Baseball, attending BHS, or zoned to BHS.

All players participating in the 2026 SAC Camp must complete this form.

A current up to date Health Physical is required to participate in Bellaire Baseball SAC Camp. HISD physicals are good for one year from the date they were signed by your health care practitioner.

If you are a current BHS student with an current Participation Physical on file, you do do not have to resubmit.

If you are an in-coming student or a current student with an expired HISD physical you will need to submit a current physical to BellaireCBBC@aol.com before the start of the SAC CAMP
