I/we , the undersigned, hereby give my/our permission for the child listed as "Player" to participate in all activities sponsored by the NEO Blue Diamonds Fast Pitch LLC softball team, coaches, and helpers. I understand that participation may result in injury, and or death and protective equipment does not prevent all injuries to participants. I do hereby waive, release, absolve, indemnify, and agree to hold harmless NEO Blue Diamonds Fast Pitch LLC, facility owners , field owners , coaches , volunteers and participants from any injuries or property damages that may result during the participation of activities sponsored by NEO Blue Diamonds Fast Pitch LLC.
By completing this 2021 registration form, you are accepting a spot on a Blue Diamonds Fast Pitch, LLC team for the Summer 2021 Season. You agree that your daughter will be committed to the team, and to always show good sportsmanship as a parent. Further, to follow and abide by all rules and by-laws of the organization. You agree to respect coaches, players, other parents, and other teams. You agree to pay fees out of pocket, or to participate in our fundraising events to cover all costs associated with playing on our teams. All fees must be paid in full before any player may play in league or Tournament games. 2021 Registration fee to secure a spot on your team is $100.00 and is Non Refundable. This amount is due within one week of acceptance or your roster spot may be offered to another player-
The total base annual player fee for 2021 is $450.00 which is broken down into the initial $100 deposit, and 1 additional $150 payment due by the end of November, 2020. The remaining $200 is intended to be paid thru team fundraising, and or team and player sponsorships. Goal is for parents to pay no more than $250 out of pocket for the 2021 season. Should a team fundraising shortfall occur the balance to get each family to $450 will be paid out of pocket. If a family decides not to participate in team sponsored fundraising you agree to pay $200 additional to get your player fees up to $450.00 You agree to pay these fees in a timely manner as designated above. Please discuss with coach any special financial circumstances. For Players signing up later in the fall of 2020 or winter 2021 Initial $150 payment is due at time of registration to begin practicing, then $100 additional one month later followed by team fundraising or additional payment of $200 over the next 2 months to get player fee up to $450. Annual fee has to be paid in full prior to the start of the 2021 outdoor season to participate in games.


For the 2021 season there will NOT be a mandatory organizational fundraiser (Daily Number) as we have traditionally done to cover the cost of our indoor practice facility available to all of our teams on a year round basis. We may resume this in future seasons. Your team may offer additional fundraising opportunities.

Lastly, all players/parents will be asked to volunteer a couple of hours of their time throughout the season or in late August to assist at our home field of SJB with their festival. Typically we assist with parking detail in return for use of facility/practice field, etc. All families will be expected to assist with this event by donating a couple of hours of time in August 2021 for the prior season. Each Team is expected to donate 24 man hours of time. 10u players are too young to assist with this obligation, so it will be parents, older siblings (9th grade or older), grandparents, etc. who will need to fulfill each family's 2 hour obligation.
