WAIVER INFORMATION

The player understands and agrees that:
a) I agree to play with team assigned and no other in the Men’s Competitive Hockey League.
b) I will abide by the league rules and regulations so adopted.
c) I play in off shift time. Time off from work will not be granted in order to participate in games.
d) Connection with team activities ceases if employment is terminated by ArcelorMittal Dofasco.
e) Helmets and protective face masks are mandatory. Players will not be able to play without a minimum of a half face shield. Full CSA Approved equipment is required in all games.
f) Is responsible for any property damages resulting from his own negligence.
g) Can have all privileges revoked immediately, at anytime, at ArcelorMittal Dofasco’s sole discretion.

CONSENT AND RELEASE
I understand that I participate(s) in these activities at my own risk and there shall be no liability whatsoever attached to ArcelorMittal Dofasco, its servants, agents, employees, other participants, or independent contractors for any loss, damage or injury of any nature, however caused. In case of any medical or other emergency that may occur while I am participating in these activities, I hereby authorize ArcelorMittal Dofasco, its servants and employees, to take such action as they consider appropriate, including, without limitation, providing or obtaining medical or surgical attention. Any personal information collected, used or disclosed, is solely for the purpose of administering the Recreational Programs for you and your dependants.
