Player Registration Form.

This form must be completed by each player and filed with the League forty-eight hours (48) prior to
the game (this includes the first game of the season), before a player can play in a scheduled League
or Cup game. If any player plays a scheduled League or Cup game without first completing and filing
this form with the League, he/she SHALL be suspended from further play for the remainder of the
season, and the team using the ineligible player will forfeit their deposit, their most recent Challenge
Cup decision and/or any points awarded from League games in which the player has played.

WAIVER AND RELEASE

In consideration of the Golden Triangle Old-Timers Soccer League allowing me to be a
member/player, I (or my proxy , manager or otherwise) waive and release the Golden Triangle Old-Timers Soccer League management
and ALL other organizers and representatives of the League from any claim for any injury I may suffer
arising from my participation in League and Cup games, whether the result of negligence or
deliberate act, or the condition of the playing field, or any other cause.
I confirm that I am in proper physical condition to participate in the Golden Triangle Old-Timers
Soccer League, and I am aware that participation could, in some circumstances, result in physical
injury.
