Player Registration 2018 Lacrosse Season

Important! Please Read!

Besides a players pads and lacrosse stick, all players are required to have a team helmet and uniform. Please check players head size and order the appropriate size helmet. Many 10-12 graders like the CPX-R but most all players except those with larger heads can wear a CPV-R helmet. CPV-R and CPX-R helmets look similar and are acceptable for both Middle School and High School games. Team helmets are an investment and can be worn for multiple years.
All players are also required to purchase their own uniforms. Middle school and High School uniforms are different but players can use re-use the same uniform as long as they are on the Middle School or High School team respectively. Once issued, a player will keep the same number until he requests a different number or changes from Middle School to High School. We try to keep players numbers the same through Middle and High School but number requests are based on a first come first serve and seniority basis. New 9th grade players should order a High School uniform. If you qualify to play U-15 as well a uniform will be loaned to you for those games.

WAIVER INFORMATION

The game of lacrosse is a physically demanding sport and there are inherent risks of injury in playing the game either through player/player contact, player contact with lacrosse balls and lacrosse sticks as well as general injuries related to playing any contact sport in general.

I, the undersigned, desire to voluntarily participate in the Denver Spartan Lacrosse Program (hereinafter “activity”. I represent that I am knowledgeable of this activity and the risks of personal injury or property damage to myself and to others which may be associated with the activity. Notwithstanding these risks, I wish to assume them by voluntarily participating in this activity and in any travel associated with this activity during the 2015 - 2016 academic year.
I understand and agree that Denver Lacrosse Program accepts no responsibility for my acts or the acts of others while I am participating in and traveling in connection with this activity.
In consideration of Denver Spartan Lacrosse Program offering this opportunity and allowing me to participate in this activity, the receipt and sufficiency of said consideration being hereby acknowledged, I hereby do release, relieve, discharge and hold harmless Denver Spartan Lacrosse Program, its officers, trustees, employees, and representatives from any and all liability, whether for personal injury, property damage, or otherwise, arising out of or in connection with participation in this activity and any travel associated with this activity.
