WAIVER INFORMATION

RELEASE OF LIABILITY FOR MINOR PARTICIPANTS:

IN CONSIDERATION OF my child/ward, being allowed to participate in any way in the SUMRALL GIRLS SOFTBALL LEAGUE related events and activities, I acknowledge, appreciate and agree that:

1. I for myself, my spouse, and my child/ward, am VOLUNTARILY PARTICIPATING in the Sumrall Girls Softball League (“Sumrall GSL”) programs, events and activities; and,
2. The risk of injury to my child/ward from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
3. I voluntarily for myself, my spouse, my child/ward and on behalf of my/our heirs, assigns, and next of kin, HEREBY RELEASE AND HOLD HARMLESS Sumrall GSL and its directors, officers, officials, coaches, volunteers, sponsors, and other participants (‘Releasees”) from any liabilities, claims, damages, injuries, losses, and expenses including reasonable attorney’s fees and cost whatsoever, including those for personal injury, death or property damage, which may arise from or in connection with participation in these programs; and,
4. I willingly agree to abide by all Sumrall GSL rules and regulations, This release shall remain in effect until cancelled in writing; and,
5. I, for myself, my spouse, my child’s/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities incident to my child’s/ward involvement or participation in these programs, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.

IMAGE RELEASE:
I hereby give permission to the Sumrall Girls Softball League to use any photographs taken by the League, its officers, directors, volunteers or agents, of my child/ward during participation in this program. I agree such photographs shall be the property of the Sumrall GSL and I am not entitled to compensation of any kind for the use of such photographs. I also agree that such image or likeness of my child/ward may be published in an outlet used to promote or publicize the League.
