MEDICAL INFORMATION

PERMISSION TO PLAY AND WAIVER OF LIABILITY
I give my permission for my daughter to play on the Albert Lea Girls Basketball Boosters Association (ALGBBA) Traveling Team. I acknowledge that playing on the traveling team could lead to injuries because of factors inherent in this type of physical activity. I absolve the ALGBBA and the people associated with the ALGBBA (including, but not limited to coaches, parents, and financial backers) from any liability or blame from any and all injuries, accidents, damage which may occur during the ordinary course of the activity. If any injury occurs that requires the assistance of a physician, I give my permission to the coaches to seek proper medical treatment.

PERMISSION TO PUBLISH
The law requires that we ask for your permission to use information about your child. ALGBBA requests permission for your daughter's photo/image, and personally identifiable information, including jersey number, first and last name, and grade level to be published on the ALGBBA website, Facebook page and/or Albert Lea Tribune.

REGISTRATION FEES
I agree to pay the designated registration fees based on 2015-16 grade level:

3/4th grade - $75
5-7th grade early registration (before 6/1/15) - $125
5-7th grade registration (after 5/31/15) - $150

UNIFORM DEPOSIT
I agree to pay a refundable uniform deposit of $100. The deposit will be returned at the end of the season (3/31/16) if the uniform is returned in good condition.

VOLUNTEER COMMITTMENT
I agree to pay a $150 volunteer deposit. This deposit will be returned after the season (3/1/2016) if our family has fulfilled all volunteer requirements. A finalized volunteer commitment form will be available before the start of the season. A minimum of 15 hours of volunteer time will be required of each family.

ALGBBA PHILOSOPHY
Albert Lea Girl’s Basketball Booster Association Philosophy

The Albert Lea Girl’s Traveling Basketball Program is designed to offer girls in grades 4-7 an opportunity to learn and develop skills vital to the game of basketball. Participants will be offered the chance to play basketball on a developmental level. Our purpose in this undertaking is to develop a program where players will develop consistency in the skills and knowledge vital to the success of the entire basketball program in Albert Lea.

At the 6th and 7th grade level, there will be more emphasis on playing to win philosophy, but not to the degree to win at all cost philosophy.

The Albert Lea Girl’s Basketball Booster Association philosophy is to provide an atmosphere of respect, learning, and most of all fun for all girls involved.

As a willing participant in the Albert Lea Girl’s Traveling Basketball Program, the following rules and guidelines are applicable to each player, coach, and parent/guardian involved in the program.


Players

All girls attending a public or private school located within the boundaries of School District 241 are eligible to participate.

1. Players are expected to respect each other at school, practices, games, etc. Teams are encouraged to attend and support other Albert Lea teams when possible.
2. Teams will be broken down equally depending on the number of players. There may be an A and B team if enough players are available. This will be at the coach’s discretion.
3. Attendance is required at all practices. Practices are as important as the games. If a player misses practice, playing time will be limited at the coach’s discretion. If a player must miss or has an emergency, the coach must be notified. Injured players should attend games and practices if possible so they can continue to observe different drills.
4. All players must be picked up on time at the end of practice. Due to liability issues, students should not be left in facilities unsupervised.
5. All participants will adhere to the District 241 polices involving academics, chemical abuse or behavioral suspension. If a student has been suspended from school or the school basketball program, that same student shall also be suspended from the Traveling Basketball Program. Poor sportsmanship and foul language will not be tolerated. Failure to follow the appropriate etiquette and rules during practice or games will result in the player being suspended from that practice or game and any further disciplinary actions that are deemed necessary.
6. All players will wear the required uniform, socks, and shoes. The Booster Club supplies the jersey and shorts. Uniforms will be distributed to the players at the start of the season and collected at the end of the season. If the player loses the uniform there will be a replacement fee charged to the player. This fee must be paid in order for the player to continue with basketball.
7. All players are required to sell burger basket tickets for the annual fundraiser or pay the cost of 10 tickets.

Coaches

1. There will be designated coaches for teams. Interested individuals may volunteer or the booster club will seek out people to coach.
2. A coaches meeting will be held at the start of the season for all coaches.
3. Both team coaches are expected to work together if there are two teams in a grade.
4. Practices will be combined if there are two teams for a grade level. There will be no separation of teams during practice. Scrimmages at practice can be A team vs. B team or mixed depending on the focus of the practice. Practices will be scheduled by the Basketball Boosters depending on the availability of the gyms.
5. Both teams will run the same offenses.
6. Practices will consist of running and conditioning, with no standing around. The coaches will have the players work on fundamentals such as passing, dribbling, shooting, defense, etc.
7. During any given game the players in the 4th, 5th grades will have equal playing time in the first two thirds of the game. In the last third the coaches will have discretion for substituting players as deemed necessary to win. Players will not be allowed to play an entire game without any substitution unless there are not enough players to substitute. No player will be allowed to sit out an entire game unless an eligibility requirement has been violated.
8. When rotations and substitutions have been set and scheduled by the coach, substitute coaches are expected to follow the pattern.
9. Basketball is a team game. There will be no emphasis regarding leading scorers. Coaches are expected to work on creating role players, assists, rebounds, steals, blocks, defense, etc.
10. If a team is missing players, players must be brought up or down to help when they are available. When A team players play on the B team, they will not start or take away quality playing time from the B team players.
11. If there are two teams for a grade, sharing or floating of players must be done evenly throughout the team. One or two players will not consistently be the floaters or take playing time away from the other team members.
12. The Booster Club sets aside money for off-season opportunities. When using the booster money, it is recommended to include as many players as possible. Formation of any team must consist of at least 8 players on the roster. Coaches should encourage further participation by all the players to participate in summer camps, AAU, etc.
13. The Booster Club and coaches will choose tournaments. No tournaments can be entered without Booster Club approval. The approximate number of tournaments for 4th, 5th, and 6th grade will be six and 7th grades will be seven.
14. If the coach cancels a practice gym time, the coach is responsible to contact the Girl’s Booster gym time coordinator so the facility can be used for other requests and the custodians are not scheduled to work when the facility is not being used. The Booster Club must approve and schedule all gym time that the Booster Club is expected to pay for.
15. Background checks will be done on all volunteer coaches.

Parents/Guardians

1. Positive attitudes will be shown at all times by parents, coaches, and players.
2. Everyone must be role models and represent Albert Lea positively. No yelling or degrading players, coaches, referees, or other teams.
3. Parents are to pick up their child on time after completion of practice.
4. Parents and players are required to volunteer and participate in fundraising activities.
