I/we agree and acknowledge that my child’s participation in the personal training conducted by FC Brashear Training poses the potential risk of injury to my child. For and in consideration of the opportunity for my child to participate in such training activities, I hereby voluntarily release, discharge, waive, and hold harmless FCBTraining and its staff from any and all actions or causes of action for personal injury, bodily injury, property damage, or wrongful death occurring to my child arising in any way whatsoever as a result of my child’s participation in such training activities. I furthermore agree to indemnify and defend FCBTraining against such claim as a result of my child’s participation in the above described personal training.
