The undersigned parent/guardian of the above named player, who is a candidate for a position on a NNLL team, hereby gives my approval to their participation in any and all activities of NNLL during the current season. I assume all risk incidental to the conduct of activities and transportation to and from activities. I hereby release, absolve, indemnify and hold harmless NNLL and the organizers, sponsors, supervisors, board of directors, and coaches, from any and all liability associated with such activities. NNLL reserves the right to rescind playing privileges if conditions warrant the same, and the decision of the NNLL board of directors is final in that regard. I will furnish a certified birth certificate at the time of registration.
