WAIVER INFORMATION

On my own behalf and as parent or guardian, and on behalf of, the minor registering for and participating in this spring/summer football program, both the practices the subsequent games or activities operated by or participated in by Yorkton Minor Football, acknowledges that football is an inherently dangerous sport in which injury of varying degrees of severity, up to and including death, is common and that the intention of this document is to assume risk and waive and release claims of liability.
On my own behalf and on the behalf of the minor registering and participating in this spring/summer football program and any such subsequent games or activities, I hereby waive and release any claim whatsoever against Yorkton Minor Football, as well as any officers, directors, coaches, managers, trainers, volunteers associated with Yorkton Minor Football for any claims, damages, injuries or losses to personal property and/or for personal injury or death arising out of the participation of the minor in this spring / summer football program and any games or activities they participate in as a member of the football program operated by Yorkton Minor Football. On my own behalf and on behalf of the said minor, I also agree not to bring any claim against a third party who may in turn claim against Yorkton Minor Football, and/or its officers, directors, coaches, managers, trainers, volunteers associated with Yorkton Minor Football Team, including for contribution or indemnity. I have read and understand this Assumption of Risk, Waiver and Release of Liability.
