WAIVER INFORMATION
Uintah Youth Football Consent Agreement

I consent to participation in the Uintah Youth Football, Inc. program. In consideration for participation in the activities conducted by Uintah Youth Football, Inc. I release, waive, discharge, covenant, and relinquish any and all action or cause of action against the organization, promoters, officials, staff coaches and/or volunteers for personal injury, death, and or property damage occurring as a result of engaging in the activities conducted by this Organization. I further release all officials, officers, promoters, staff, coaches, and/or volunteers from any claim whatsoever on account of first aid, treatment or services rendered during participation in this organization. Any intentionally misleading information provided in this form may result in being disqualified from participating in Uintah Youth Football, Inc. events for the remainder of the season. I understand that if I cannot or will not complete the duties assigned to me I may be relieved of my position by the remaining majority of the board of directors. I further agree that any and all rules put in place by the majority of the board of directors will be followed. I understand that I must complete a national background check, that will be reviewed by the current board of directors, before I will be considered for a position on the Uintah Youth Football, Inc. Board of Directors at my own expense. Board Meetings will be scheduled monthly. By not attending 2 consecutive scheduled meetings, I understand that it may be voted on by the remainder of the board that I will be terminated from The Uintah Youth Football Board of Directors. In addition to scheduled meetings, immediate issues, may be addressed by Phone, Text, and/or Email. Answers must be given within 24 hours, or it will be known that I have abstained from the vote. Issues that may arise that are more serious in nature, will have to be discussed in a board meeting, with as little as 1 day of notice, to all board members. Board members not in attendance will be considered, abstained from voting. Coaches serving on the board may not place a vote pertaining to another coach in their division.
If a spouse or family member of a board member, coach or player holds a place on the board, issues pertaining to that board member, coach or player specifically, may not make decisions pertaining to the issue.
By signing below, electronically, I understand it represents the same signature as a hand written signature.
Uintah Youth Football Waiver of Notice:
By signing below, I have been advised of the following:
1) Board Meetings will be held in The Community Room located in the Vernal City Bldg. from 6:00- 9:00 pm. All regularly scheduled meetings will take place on the 2nd Thursday of each month, Unless otherwise notified. All meetings are closed to the general public.
2) I may be contacted to attend additional "Emergency" meetings with as little as 24 hours notice. Issues that may arise that are more serious in nature, will have to be discussed in an Emergency board meeting, with as little as 24 hours' notice. Board members not in attendance will be considered, abstained from voting, unless they have submitted their vote via text to the secretary.
3) By not attending 2 consecutive scheduled meetings, I understand that it will be voted on by the remainder of the board that I be terminated from The Uintah Youth Football Board of Directors.
4) In addition to scheduled meetings, immediate issues, may be addressed by Phone, Text, and/or Email. Answers must be given within 4 hours, or it will be known that I have abstained from the vote.
5) Coaches serving on the board may not place a vote pertaining to another coach in their division.
6) If a spouse of a coach holds a place on the board, issues pertaining to that coach specifically, may not make decisions pertaining to the issue.
7) All Board Members are required to attend board meetings, complete their assigned duties in a timely fashion, be present and help at all UYF events, assist in equipment handouts & returns, and the annual equipment shed cleanup, inventory and repairs.
8) If a board member needs an issue placed on the agenda it must be submitted to the secretary, 48 hours' prior to the regularly scheduled board meeting.
