WAIVER INFORMATION

Uintah Youth Football, Inc. is a youth organization, and all decisions made concerning coaching staff will be made based on the best interest of the players and league as a whole.
1. Uintah Youth Football, Inc. is not obligated, to appoint all applicants to the coaching staff.
2. All applicants will be required to pass a national background check every two years. Instructions for completion, will be sent via email. The cost is $19.95.
3. All Head Coach applicants will be required to declare the name of their prospective Assistant Coach to the selection committee at or before the interview process. The assistant coach will be required to pass a background check. If an Assistant Coach cannot be declared, the coach selection committee, can appoint one at its discretion, prior to the seasons start.
4. All Head and Assistant Coaches are required to be interviewed by The Director of Football Operations/Coaching Director and its Sub-Committee.
5. All Head and Assistant Coaches will be required to attend a mandatory Coaching Clinic. Date is to be determined.
6. All Head and Assistant Coaches are required to be heads up trained and be able to properly evaluate players for concussions and follow concussion management protocol.
7. I understand that if I am appointed as a volunteer on the coaching staff, I will uphold all the rules and regulations of Uintah Youth Football, Inc.
8. I understand that any materials provided to me for use during the season are the property of Uintah Youth Football, Inc., and I am responsible for the matter in which they are used.
9. Player and parent information in rosters is considered confidential and cannot be used or shared outside of the UYF organization unless express written permission has been granted by the Uintah Youth Football Inc., Executive Board of Directors.
10. Materials containing the UYF logo are the property of Uintah Youth Football, Inc. and cannot be used outside of the organization for any purpose unless express written permission has been granted by the Uintah Youth Football Inc., Executive Board of Directors.
Participation in the Uintah Youth Football, Inc. program is to be considered a privilege, not a right.
In consideration for participating in the activities conducted by Uintah Youth Football, Inc. I release, waive, discharge, covenant, and relinquish any and all action or cause of action against the Uintah Youth Football, Inc. organization, promoters, officials, staff coaches and/or volunteers for personal injury, death, and or property damage. I further release all officials, officers, promoters, staff, coaches, and/or volunteers from any claim whatsoever on account of first aid, treatment or services rendered on my behalf during participation in this organization.
Any intentionally misleading information provided in this form or the interview process, may result in refusal of participation in all Uintah Youth Football, Inc. events for the remainder of the season.
