Waiver
I give my approval and consent to the participation of the player listed above in the Jasper County Boys and Girls Park, events and activities. The electronic signature below acknowledges and agrees that: I am not aware of any injury, illness or other health related issues that would restrict my child's ability to play in competitive sports. I agree to assume all risks and expenses due to an injury that may occur as a result of my child's involvement in competitive sports' practices, games, and/or travel to and from said activities. I agree not to hold the Jasper County Boys and Girls Park or anyone acting on its behalf either as a coach, a coach assisting, or board members responsible in the event of an injury to my child while participating under the supervision of the above.

Medical Release
I hereby give my consent for medical treatment deemed necessary by physicians and/or ballpark authorities including, but not limited to, calling 911 and/or providing transportation to a hospital emergency room or urgent care facility for treatment of any illness or injury while at or participating in activities related to Jasper County Boys and Girls Park.

Photograph Release
I agree that Jasper County Boys and Girls Park may use photographs of my family for any lawful purpose, including, but not limited to their web-site, Facebook page, twitter account and/or cash calendar.

Conduct
Jasper County Boys and Girls Park has been fortunate in that the behavior of our players, parents and fans has been overwhelmingly positive. To maintain this standard, the board reserves the right to exclude from park attendance ANY child or adult who is a danger to himself or others, or who displays inappropriate/offensive conduct towards others.
