CERTIFICATION OF GOOD HEALTH

Wrestling is a competitive sport and training can be physically and emotionally demanding. By registering my child in the Branchburg Wrestling Club wrestling program, I certify that they are healthy, in sound physical condition, and have no ailment or organic defect that would make participation in the activity dangerous to their health. Please list any physical, medical, cognitive, and/or emotional conditions that the coaches should be aware of before participating in wrestling (i.e. asthma, ADHD, autism, etc.). COVID-19 WAIVER

I agree to not put my team and coaches at risk by bringing my wrestler to a practice or competition when they are not feeling well. I understand that even with following safety protocols my wrestler may contract COVID-19 and I hereby release Branchburg Wrestling Club and all Club officers, coaches and volunteers from any and all liability.

ASSUMPTION OF RISK, INDEMNIFICATION, HOLD HARMLESS, AND WAIVER OF LIABILITY

As the parent and/or legal guardian of the above-named minor ("Participant"), I hereby consent to his/her participation in the Branchburg Wrestling Club ("BWC") wrestling program. I understand and appreciate that the sport of wrestling carries a risk of serious injury, including death, and assume these risks in consideration for participation in the BWC wrestling program. As the undersigned, I agree to release, discharge, indemnify and hold harmless the BWC, its officers, coaches, volunteers and wrestlers from all suits, claims, or demands of every kind and character by or on behalf of the Participant or any third party arising out of the Participant's involvement in any activities with the BWC. I hereby waive and release the BWC, its officers, coaches, volunteers, and wrestlers from any and all liability for injuries incurred by the Participant while involved in activities of the BWC.

REGISTRATION FEES

Intro To Wrestling Clinic Only

$25 per wrestler

Full Season (Oct. clinic included at no cost)

$125 1st Sibling, $100 2nd Sibling, $75 3rd Sibling ($300 Family Max).

If you participate in the Intro to Wrestling Clinic during October and decide to join us for the full season, then you can deduct the $25 registration fee per wrestler when paying for the full season.

These registration fees help us cover insurance, league fees, referees, tournament fees, and team banquet.

If you have a financial hardship, please reach out to VP/Treasurer Brandon Holderbaum, or Head Coach Frazee Sutphen at branchburgwrestling@gmail.com. We have financial assistance available. We welcome all families into our club!

Uniform/Warm-Up Policy

We require all wrestlers to wear a Branchburg Wrestling Club sweatshirt, t-shirt and bottom (shorts or sweatpants) to all matches and tournaments this year. If you do not own warm-ups and/or your wrestler has outgrown their warm-ups from last year, please order via the team online store which will be linked on the website and sent via email to all registered wrestlers. Warm-up purchases are not required if you are just joining us for the October Intro to Wrestling clinic.
