I hereby authorize Air16 Bay Area NFL Flag, Instructors, and the staff to act in their best judgement in any emergency situation requiring medical attention. I agree to waive and release Air16 Bay Area NFL Flag, Instructors and the staff from any and all liability for any injuries, losses or damage while participating in flag football. I further state that the participant above is physically able to participate in playing flag football.

I also grant permission for my player's image to be used in print, video, and digital media. A agree that these images may be used by Air16 Bay Area NFL Flag for a variety of purposes and that these images may be used without further notifying me. I do understand that my player's name will not be used in conjunction with any video or digital images.
