WAIVER INFORMATION

RELEASE STATEMENT: I the Parent/Guardian of the registrant, a minor agree that the registrant and I will abide by the rules of the Saint Pius X Athletic Association (SPXAA) and the CYO. Recognizing the possibility of physical injury associated with athletic activities and in consideration of SPXAA accepting the registrant for its programs and activities, I hereby release, discharge, and /or otherwise indemnify SPXAA and its affiliated organizations and sponsors, their employees and associated personnel, including the owners of the fields and facilities, coaches and any other volunteer persons used for the programs, against any claim by or on behalf of the registrant as a result of the registrant's participation in the programs, and/or being transported to or from the same, which transportation I hereby authorize. I/We understand and acknowledge that not all children trying-out for a position on the team will be selected and agree to conduct ourselves as responsible fans without abusive language or manner towards players, coaches, fans, and officials.
