Please check the appropriate boxes for the team(s) your player will be on for the away game meals and snacks, depending on what team your player is EXPECTED to be on. We understand that the player may move teams during the year. The fee will not be adjusted if the player switches teams.
All items in between the horizontal lines are optional. The only item that is not optional is the meal and snack for your player(s). You only need to check the boxes for the optional items if you want them, otherwise leave them blank. If you do want the optional items, remember to put the quantity you want in the field below the item's checkbox.

Finally, add up the total cost for all items purchased and enter that amount in the Total field, then click the Submit button.
Please bring your checkbook to Viking Preview night and be prepared to write a check payable to "TRI-VALLEY FOOTBALL PARENTS" for the total and give it to Diana Woodring at the table outside the locker room door.
