NJ Scorpions Registration Form USA WRESTLING FEDERATION MEMBERSHIP As a USAW Federation sanctioned club, it is mandatory and required that all members of the Scorpions Training Center, LLC obtain, maintain and provide a current USAW Federation Membership card number before commencement of training. The USA Federation membership allows all members to enter into all USAW sanctioned events and is also used as SECONDARY SPORTS ACCIDENT INSURANCE. All members of the training center may go to www.themat.com to purchase a USAW Federation Membership. Our staff at Scorpions Training Center, LLC can assist you in purchasing a membership at the school as well. To date, the membership fee is $41/year. USAW membership expires August 31st of each year regardless of when purchased. SCORPIONS TRAINING FACILITY MEMBERSHIP TERMS * Members are obligated to pay membership options in full whether term is completed or not. * Membership cannot be put on hold for any reason or transferred among members. * Returned checks incur banking institutional fees. * Late Payments are subject to a $25 fee. * All fees paid are non-refundable. |
