| Makeup forms are at the front desk. Please pick one up and place your order with Faith Johnsen... All the information is on the order form. She will be taking orders until October 22nd.

| The make up forms are at the front desk and i will be there to take orders and answer any questions that anyone may have. Please remember it is cash or check and the makeup CANNOT be put on or paid through your Ultimate account. All checks as per on the flyer can be made out to myself "Faith Johnsen".
