Registration Sports Form
Divisions: Abbreviation - Little League Instructional Girls (4th grade only - LLIG), Little League Girls (LLG), Little League Instructional Boys (4th grade only - LLIB), Little League Boys (LLB), Junior Boys (JRB), Senior Boys (SRB).
Roster Form CSV
Must stay in a csv file. All columns must be filled out. List all players and coaches on your teams' rosters. List of divisions: LLIG, LLG, LLIB, LLB, JRB, SRB. Before submitting rosters make sure all players are from your elementary school district.
Background Volunteer Coaches Application
All coaches must fill out a background form to be eligible to coach a team in the Davidson County Sports League.
Bullying, Hazing and Inappropriate Behavior
All coaches are required to turn in an ABUSE AND TRAINING CERIFICATE. This is a "FREE" course. Coaches are to submit a copy of their certificate to their program director once completed.

Booster Club Player Release Form
No player can play outside their high school district. A player release form must be filled out by both booster club basketball directors or Booster Club Presidents.
