Recertification Form
Every cheerleader must have this form filled out, signed by a parent and returned to the nurse

Physical Form
If doctor is performing physical, every cheerleader must have him/her fill out, sign and return to school nurse

Parent Permission Form
If school nurse is performing physical, this form must be filled out and signed by parent. Cheerleader must bring it to physical with nurse
