Florida High School Physical Form
Every athlete is required to complete a Florida High School Sports Physical prior to the season. (Please be sure to have the doctor sign, stamp and date the document) A school entry physical is not the proper document and will not be accepted during registration.- Business Sponsorship Form
Each year we provide local businesses an opportunity to sponsor our program and athletes directly. No amount is too small or large. If you would like to support a specific athlete or service in our program please be sure to reflect that on the form and payment memo.
