  • Concussion Awareness Form
    In compliance with the Maryland Youth Concussion Legislation (2011), each youth organization within Anne Arundel County is required to complete and keep on file the Concussion Awareness Form for each player.

  • Youth Player Release
    "If a participant in a county-sponsored youth league wants to change organizations (by sport) from the previous
    season, a Youth Player Release Form must be completed and submitted [with their Youth Player Contract]."

  • Youth Player Contract
    Parents/players who wish to play county b-ball must complete, sign, and turn in this form to their coach by a certain deadline (TBA).
