All players must complete this clearance and have a current sports physical on file before participating in any team activities for the Fall. Don?t wait, get this done early to avoid a last-minute rush before the season begins. - Eligibility Requirements

CAMP WAIVER - DUE FIRST DAY OF SUMMER
The camp waiver is required to participate in any summer football activities, including conditioning and lifting sessions. Please bring the completed form along with your camp contribution on the first day of camp, players will not be allowed to participate without it.- Online Pre-Participation Physical Evaluation
