List of Required Forms - Tackle Football
The following items are all REQUIRED to be submitted prior to equipment handout.
**No forms - No equipment - No Exceptions**
Any hard copy forms can be given to your respective Team Director during July conditioning or August practices.
2019 Medical Release Form (Required)
This form is required to be submitted when registering. Please have your doctor complete the bottom portion and submit along with a recent physical (less than 1 year old). Return completed form to your Team Director.
Image Release Form (Required)
This form is required to be submitted when registering. Please print, read, sign and return to your Team Director.
Parent/Athlete Concussion Form (Required)
This form is required to be submitted when registering. Both Parents/Guardian and player must sign this form. Please print, read, sign and return to your Team Director.
