AYF Participant Packet
To be officially rostered, each player must submit a completed AYF packet.

In addition to the AYF packet, the following documents are required:

- Physical dated in 2025. The doctor must complete the medical clearance form included in the AYF packet (page 5). No other physical forms will be accepted.
- Copy of the player?s birth certificate.
- State-issued ID for the player.
- Passports will not be accepted.
- Military IDs are accepted, but a state ID is strongly recommended since it must be retained for the entire season.
- Copy of the player?s most recent report card.

All documents must be submitted before a player can be officially placed on a roster.

Medical Clearance Form
This is the only form that will be accepted for your player or cheerleader's sports physical. It can also be found on page 5 of the AYF Participant Packet.
