You can fill this form out on your computer then print it.
You cannot save the information you entered on this form.
Once the first child is printed, just change the pertinent information
(name, birthdate, medical history, etc.) for each additional child,
then print the form again.

PLEASE FILL OUT MEDICAL INFORMATION SECTION AND INITIAL. (It gets over-looked often)

In the COMMENTS BOX, please put your child's three number choices (remember, these are not guaranteed)

Do not fill out the area's marked 'For Club Use Only' or areas in dotted boxes.

If you want to work in the concession stand please mark the box on the registration form. If you do not mark that box you will not be called to work the stand.
** Remember, you will receive your $25.00 concession stand fee back upon working in the concession stand ONE TIME.**

Registration Fee for each child
Beef Raffle Fee for each child
Copy of Birth Certificate (Only if 1st year player)
Shirt size (have three numbers in mind, numbers are not guaranteed)
Pant size for boys & girls
