REGISTRATION REQUIREMENTS
Please have your child present at the time of registration
Parent/Guardian will need to complete a registration packet
Player/Athlete will have to take a photo for a Conference-issued Player ID Card
Player/Athlete will have to complete and clear a physical examination to participate in the program
PLAYERS/ATHLETES will need the following: original birth certificate, original report card, copy of utility bill as proof of address and copy of their medical insurance card.
BREAKDOWN OF REGISTRATION COSTS
Uniform Game Jerseys (x2) (Home & Away)
Any and all practice field and game day usage fees
Any and all light and facilities fees
Any and all league fees
Any and all league insurance costs
Any and all referee, medic and game fees
Player trophy and banquet ceremony
MANDATORY PARENT/GUARDIAN REQUIREMENTS
Parents/Guardians MUST participate in a MANDATORY fundraiser of selling raffle tickets (60 tickets at $5.00 each for a total of = $300.00)
Parents/Guardians MUST volunteer a minimum of (2) hours of their time for any given event
REFUND POLICY
No refunds for disciplinary reasons, ineligible players, including injuries, academic grade checks, and probation.
Exception for a refund, only if there is no team to fill a division and/or the city does not have a season.
