1.) Original Birth Certificate (with 2 copies)      2.) Physical on 2017 Physical form

3.) Report Card (If want to be considered for Scholar Recognition)

Forms needed: (Printable)

1.) League Contract       2.) Parents Code of Conduct

3.) Physical on 2017 AAU form                 4.) Background Release (for all Staff / Volunteers)

5.)  *2017 AAU Contractt (* Reference only, Original will be given at time of Sign-up)

ATTIRE NEEDED:

Football:

White T-shirt with last name on back  and black shorts,

Mouth Piece with strap (no clear, red, or white)

Girdle, Cup and Cleats

Cheer:

T-shirt, Comfy Shorts or leggings, tennis shoes,

towel and hair pulled back in ponytail
