Dear Parents,
I am excited to announce that OHS has decided to partner with FinalForms on creating a better experience to submit athletic participation forms. From this day forward the only paper form that will still need to be turned in will be the OHSAA Pre Participation Physical pages 1-4. All other forms will be reviewed, signed, and submitted electronically.
“Registering your first child will take 5-10 minutes. Registering additional children will take 2-3 minutes each. Information is stored from season-to-season, year-to-year and also shared within families eliminating the need to enter the same info more than once. Your only annual requirement will be logging in to verify the existing information for your child by re-signing each form.”
As you can see, this system will be saving time and energy for all parties involved and will be a major asset to the OHS Athletic Department. One other great perk to this system is that you, as parents, will receive automatic email notifications that your son or daughter's physical is about to expire.

Completed Physicals should be e-mailed or delivered in person to the Athletics Office or to Lilian Robinson prior to the first day of tryouts.
