New Parents/Athletes: Take a moment to answer the following questions. Email your responses to fsathletics@yahoo.com.
Type 2026 Inquiry in the subject line.
1. Parent Name, Contact Information(email, home or cell phone)
2. Child Name, Age, DOB, Grade, School Attending
3. Does your child compete in any organized competitive athletics?
a. If yes what sport?
b. How long?
c. What team?
4. Has your child competed in track and field?
a. If yes, what events include results, and track meet date.
5. Is your child currently a member of another Youth Track and Field Club
a. If yes does your head coach know you are not returning? Yes or No
Athlete Specific questions:
1. Why do you want to run track and what are your goals for the season?
2. Why are you interested in running with Future Stars Athletics?
Please Note:
All Future Stars Athletes are required to compete in the following Championship Track Meets: Three Rivers Associations, Regional Championships, and Junior Olympics.
The Outdoor Season Begins in April and Concludes at the End of July. We are looking for Track and Field Athletes/Families dedicated to the sport. If you cannot commit to the entire season do not complete the inquiry form.
