MGA MEMBERSHIP APPLICATION

 

 

 

Thanks for your interest in the MGA.  We have a great group of guys that play each Wednesday morning.  To get you signed up, we will need the following information from you:

·         Your GHIN Number:

 

·         What tees would you like to play?  

 

·         Would you like to play in the weekly skins game?  This is optional and would be an extra $5 per week.  (We charge $5 per player for the weekly game.)

 

·         Membership is $95.00 per year.

 

·         Your birth date.

 

·         Your home address and phone number.

 

·         Your emergency contact and phone number.
