Spring Grove Youth Basketball

All Coaches and On-Court (Practice) Volunteers must submit for their Child Abuse History Clearance and PA State Police Criminal Record Check (both free for unpaid volunteers). You MUST also either fill out the FBI Clearance Affidavit linked below or obtain the federal FBI fingerprinting clearance requirements below. 

STEP 1 (all volunteers turn in A, and B if PA resident for less than 10 years):

A. FBI Clearance Affidavit: FBI_Clearances_Fillable_PDF_Verification_Form.pdf   

• If volunteer has continuously resided in Pennsylvania for the last 10 years AND swears or affirms in the document below that he or she has never been convicted of certain disqualifying crimes in PA, or equivalent crimes under Federal Law or the law of another state, they need only obtain the child abuse and PA state police clearances above and to fill out the affidavit. Please print out the form, fill it out, and bring it along to the coaches meeting (TBA) or scan and email to sgybapres@gmail.com prior to start of season.

AND

B. FBI Criminal Background Check - Code needed for Volunteer 1KG6ZJ

• If volunteer has lived in Pennsylvania for less than 10 consecutive years, they must obtain the federal FBI fingerprinting clearance requirements above and then have their fingerprints taken at LIU New Oxford (see info below). 
  • ***The club will reimburse any coaches who need to utilize this option.

• If you have lived in PA for 10+ years, you may skip Step 1B.

STEP 2 (turn in A and B):

You are required to complete either A OR B from Step 1, and both A AND B from Step 2.
