As we prepare for the upcoming season, we ask that all players and guardians review the Green Lacrosse Club Parent & Player Handbook, including the Nondiscrimination and Anti-Harassment Policy, and complete the Annual Acknowledgment Form together. This form is essential to ensuring that all players and families understand our expectations for sportsmanship, conduct, and a respectful playing environment.
Important:
✅ The form must be completed by both a guardian and the player.
🚫 Players who have not completed the form will not be permitted to participate in games.
🚫 Uniforms will not be released until the form is submitted.
Please take a few minutes to review the policies and complete the form as soon as possible. Your cooperation helps us maintain a positive and supportive experience for all players.
