To find the Registration forms go to the ABOUT tab, then the PHOTO ALBUMS tab, they are under Registration.  Print them out fill them out and send them to the above email addresses.  Make sure to put the team name at the top of the form or "New Registration" if the player hasn't played prior to this season.
