2025-26 Fall Sports information

Good afternoon,
 
      As we are approaching the 2025-26 school year I wanted to give a brief overview of the winter sports programs at Willowick Middle. If your child is participating in a winter sport, please read the following carefully:
 
    1. Every child must have a current physical in order to participate/try-out for a sport. There are no exceptions to this rule.  This is a state rule, as the school is liable for all athletes who are practicing after Aug.1. Attached is the physical form that the OHSAA uses. A signed copy by the doctor needs to be uploaded into final forms or a copy is to be given to the coach on the first day of tryouts or practice. For a cut sport such as volleyball, a student will not be able to try out until she has a physical on file. If you haven't had a physical yet you may have to go to a minute clinic to get a spot as doctor's offices get backed up this time of year. All physicals are good for 13 months from the day they get them. The following link will take you to the physical form:
 
     2. You must register your child in final forms.  Final Forms is a site that is used by the district to keep all of your child's sports information on file for the school and coaches. It is our line of communication between the coaches and families about schedule changes, practices etc. A link to sign up is on the Willowick athletic site and I will also attach the link as well. Please make sure to fill out all information accurately before the first practice/tryout. If you have already signed your child up in the past, you do not need to make a new account, but just update the sport your child will be playing for the upcoming year.      3. All student participation fees are to be paid before the first game. The district has a pay to participate fee for every sport that must be paid before the first game. Failure to pay will result in your child not being able to participate in games until that fee is paid. You can pay either on final forms or by cash or check to the Athletic Director. The fees are as follows:
$90 - If your child pays full price for a school lunch
$65 - If your child pays a reduced price for lunch
$40 - If your child is on the free lunch plan
* You must notify michael.bubonics@weschools.org if your child is on a free or reduced lunch plan in order to change the amount in final forms.
 
     
 
The following is information about practice and tryouts for Basketball
 
Basketball
Students should wear comfortable shoes and athletic attire. Please bring a water bottle.
Tryouts will be at the following times
 
Monday- Wednesday 10/13 - 10/15  
7th grade - 7pm - 8:30pm
8th Grade - 3:30pm - 5:00pm

    For the 2025 - 2026 school year, we will be using an online system to complete most required paper work for our athletes.  The program is called Final Forms. Click on the Final Forms link below to register and complete the paperwork for your student - athlete.
 
      Final Forms is an online system that will allow you to register your student - athlete and complete most required forms electronically, saving you time and less paperwork. The only form that is still required to be handed in is the actual OHSAA Physical Form that the doctor will need to sign.
