2024 Pop Warner Participant Contract and Parental Consent Form
This form applies only to the 2024 Fall to 2025 Spring season. The form must be completed, signed by the youth participant's Parent or Legal Guardian, and dated after January 1, 2024. It needs to be submitted to the West Orange Wildcats prior to participation in any programs. By signing this form, the Parent or Legal Guardian agrees to the terms and conditions outlined therein.
