Act 33/34 Clearances

Please make note that Act 33/34 clearances will be required for anyone interested in coaching this season. All information will be held confidential. If you would like additional information regarding this, please contact a board member. Clearances are good for ONE year, so you will need to obtain again this year. Please read the directions below to obtain the clearances.

The AVYFA along with the United Youth Football League requires its coaches to have Act 33 and Act 34 clearances. The forms are sent to two separate locations with separate payments. The Criminal Record Check must be filed first. Then the results of the Criminal Record Check must be sent with the Pennsylvania Child Abuse History Clearance form. The cost will be $10.00 for each clearance. The AVYFA will reimburse you the cost of the clearances when we receive the results from you. The AVYFA uses these results to approve you for coaching; the results are not the sole criteria taken in to consideration.

Please mail these as soon as possible as they take 4-6 weeks to process. Unless you have clearances from another organization, such as Little League, you must receive your clearances before you will be allowed to coach. If you have clearances from another organization, please submit a copy of the clearances we can have a copy on file with the AVYFA. A clearance is good for one year.

2) Pennsylvania Child Abuse History Clearance
