If you complete the online registration, please submit a copy of each player's birth certificate to the coach at the first practice.

In-Person Registration will take place on May 18, 2012 and June 1, 2012, from 10:00 a.m. to 2:00 p.m at the Bethel-Tate Middle School.
Registration forms will be available the day of registration or you may download the registration form and bring it with you. 
Please bring a copy of the player's birth certificate to registration.

Please mail the completed registration form, a copy of each player's birth certificate, and payment. 
Checks should be made out to BYSA.
