REGISTRATION REQUIREMENTS

 

Please have your child present at the time of registration

Parent/Guardian will need to complete a registration packet

Player/Athelete will have to take a photo for a Conference issued Player ID Card 

Player/Athelete will have to complete and clear a physical examination to participate in program 

PLAYERS/ATHELETES will need the following: original birth certificate, original report card, copy of utility bill as proof of address and copy of their medical insurance card.  

 

BREAKDOWN OF REGISTRATION COSTS

 

Uniform Game Jerseys (x2) (Home & Away)

Any and all practice field and game day usage fees

Any and all light and facilities fees

Any and all league fees

Any and all league insurance costs

Any and all referee, medic and game fees

Player trophy and banquet ceremony

 

 

MANDATORY PARENT/GUARDIAN REQUIREMENTS

 

Parents/Guardians MUST participate in a MANDATORY fundraiser of selling raffle tickets (30 tickets at $10.00 each for a total of = $300.00)

Parents/Guardians MUST volunteer a minimum of (2) hours of their time for any given event

 

REFUND POLICY

 

If a player quits before certification day, they will receive a full refund minus the $100.00 non-refundable fee. 

No refunds after jersey fitting day 

NO REFUND for any ineligible player including injuries, academics such as mid season grade checks and probation.
