The following documents are MANDATORY and are required at the time of registration or due by August 1.  NO EXCEPTIONS!!!

• All Conduct and Dicipline Forms Must Be Filled Out And Returned To Your Coach
• Medical form 
• Valid Maryland State Identification Card (this applies to tackle football and cheer) – this can be obtained from any MVA office with original birth certificate and proof of address.  Participant MUST be accompanied by their parent or legal guardian only.
