CCYA is asking for your help to recognize a deserving individual. Please follow directions on the CCYA Lifetime Achievement Award nomination form located on the "handouts" page.

Minimum Requirements of Nominee

Any CCYA program volunteer, someone that has promoted CCYA in a positive manner,

may be someone no longer involved in CCYA, and does not need to be living.

Nomination must be submitted by November 1^st of the year nominated

Board members will vote on nominees and present the Award at the

Football Awards Ceremony
