Players and Parents:

We just finished up with our five day mini-camp, We Build Champions!    Appreciate all the time and effort put into the camp as it will pay off as Monday is the first official practice with a team meeting for all 9-12th graders in UHS auditorium.

You will now have access to lockers and the locker room.  You can shower after practice and we highly encourage you to do so before leaving.   Transportation to and from Dysart UMS to LPC UHS has been arranged for young men needing a ride. If you have a young man that plans on riding the school provided transportation please text/email Coach Adam Gassman.

There are currently THREE forms that need to be done prior to practicing.  One is the high school physical which we have been communicating since starting the summer workout program.   The second one is a concussion form (handed out to athletes on Monday of mini camp) which must be read and signed by parent and athlete.   The third is a new form from UHS office.   This form can be filled out online or copy brought to the office.   Click here to access forms: http://www.union.k12.ia.us/?page_id=5459

Coach Chadd Johnson is in charge of the forms.   We are attaching the latest physical form information we have from the front office.  If there is a conflict from what is listed here please contact Brenda or Kenda via phone during normal working hours.   (342-2697)  If they are highlighted in yellow they are expired and need to be updated before they can practice.  The blue highlighted will expire during the season and there are several that expire very early in the season. 

If you are not up to date on your forms you are not excused from practice.   It is your responsibility to make sure the paperwork is finalized.
