2. For Football complete the following forms
a) Registration Form - BJT
b) Registration Form - Zero Tolerance Form
c) Registration Form - RVJT
3. For POMs complete the following forms. (Sorry all cheer squads are full)
a) Registration Form - BJT
b) Registration Form - Zero Tolerance Form
4. Include copy of birth certificate if player didn’t participate in BJT for 2011 season
5. Include payment of $170 / player
