EMERGENCY/MEDICAL CONTACT INFORMATION

Terms of Participation
1. Players in the Humber Valley Hockey Association, Murray Dryden Division (Humber Valley Old Timers) must behave in a sportsmanlike manner, with due respect for other players, officials and league executives and abide by the rules of the HVOT.
2. No body contact, no slapshots, and standard old timer rules as modified by HVOT apply.
3. The Player, for himself, his representatives, heirs, assigns and insurers (collectively, the “Related Persons”) unconditionally and voluntarily hereby Releases, Waives and Discharges HVOT from any and all, actual and/or potential claims for damages, death, personal injury (including property damage), or losses of any kind or nature to him and/or his Related Persons caused by any act, omission, decision or act of negligence on the part of the HVOT which might be associated with or result from his participation in any activity, event or hockey game organized by HVOT. The term “HVOT” in this waiver means and includes any HVHA or HVOT Executive or the City of Toronto or any individual acting through them and also acknowledges that they do not assume any responsibility for any injury, damage, or loss resulting from an incident or accident connected with participation in any HVOT activity or from any known or unknown condition, handicap, infection or from any cause whatsoever.
4. Players must monitor their own physical condition to ensure playing capability.
5. Full protective equipment must be worn at all games including CSA approved helmets. Use of full face protection is strongly recommended: half visor is the minimum accepted requirement.
6. Players must attend 60% of their league games to maintain participation in following season.
