If your player would like a trophy this season please list their name and jersey number EXACTLY how you would like it to appear on their trophy.

We are limited with the amount of characters.. middle names will be listed as just a letter if space permits.

List each player in one slot ONLY.. 1 player uses one slot...
2 players use 2 slots (one for each player) etc.
